Adults 21 years of age and older who are dealing with some sort of traumatic brain injury, developmental/cognitive disability, or autism. Schizophrenia, mood disorders, anxiety, obsessive compulsive disorders, TBI, IDD, and depression.

Residents are privately funded through trusts, disability checks or private pay. Medicare and Medicaid does not pay for our programs/facility but are used for medication and routine medical care.

  • Eating: Residents may receive gentle assistance with portion control to encourage balanced, healthy eating habits. Staff can also cut up meals as needed to promote both safety and greater independence.

  • Toileting: Friendly, timely reminders help residents stay consistent with their personal hygiene routines.

  • Grooming and Dressing: Support with choosing suitable clothing or fastening buttons builds confidence and allows residents to express their personal style.

  • Bathing: Brookside Farm offers bathing assistance tailored to each resident’s needs. This can range from simple supervision and cueing to hands-on help (no lifting or transfers involved).

  • Transferring/Mobility: Minimal assistance is provided when needed to steady movements and prevent unsteadiness.

    At Brookside Farm, our caring team personalizes this support to match every resident’s unique strengths, creating a warm and encouraging environment where individuals can thrive with exactly the right level of help. Please note that higher ADL support needs may increase costs, as they typically require more staff time, specialized resources, and individualized care planning to ensure safety and well-being.

To be admitted to Brookside Farm individuals must:

  • Be medically and psychiatrically stable.
  • Voluntarily take all medications and attend necessary medical appointments.
  • Not pose an elopement risk.
  • Not pose a threat to themselves or others.
  • Voluntarily come to live at Brookside Farm.
  • Be able to respond to emergency alarms, follow verbal instructions, and exit the facility. 
  • Be ambulatory with or without an assistive device such as a walker.
