  • Taiwan carries one of the world’s heaviest kidney disease burdens globally; however, its single-payer universal health-care system has enabled policy-driven transformation of kidney care through population-level screening, multidisciplinary pay-for-performance programmes, data-driven surveillance and digital innovation. These strategies have reduced disease incidence and improved outcomes, and provide a replicable model for health systems elsewhere.

  • The carceral system has the impossible task of providing appropriate care for a vulnerable population while limiting taxpayer spending. Although many risk factors for incarceration also increase the risk of chronic kidney disease (CKD), specific CKD screening requirements or treatment accountability are lacking, which compromises patient care.

  • The prevalence of kidney disease is increasing in children and young adults, who face a lifetime of chronic disease with considerable morbidity and mortality. However, a critical shortage in the paediatric nephrology workforce and deficiencies in paediatric-to-adult transition care mean that we fail to meet the needs of this vulnerable population. Paediatric and adult nephrologists need to take collaborative action to address this crisis.

  • The thymus is essential for T cell development and a decline in thymic health or a thymectomy therefore have profound clinical implications. Loss of thymic function might contribute to the uraemia-associated ageing of the T cell compartment, and new research is needed to clarify potential underlying mechanisms and effects on patient outcomes.
