Becker’s highlighted five “unlikely” AI champions at health systems and for anyone working in healthcare AI, this should not feel unlikely at all. Frontline clinicians know where the friction is. They know which workflows are broken. They know where documentation burden, safety risk, and operational inefficiency show up first. The opportunity is not just to give clinicians AI tools. It is to create the structures that allow them to identify problems, shape solutions, test workflows, and own the change. When we empower frontline clinicians with the right tools, support, and governance, they will deliver. Thank you, Giles Bruce, for highlighting this important work. And a special congratulations to Vickee Sevillano and our Mount Sinai Health System for continuing to show what clinician-led AI transformation can look like.

Coding agents are often benchmarked like exam-takers: given the full spec up front, then graded on the final code. But real coding help is a conversation — users clarify goals, add constraints, and correct course along the way. SWE-Together turns real coding work into a reproducible, verifiable benchmark: 109 repo-level tasks curated from 11,260 recorded sessions, replayed with a reactive LLM user simulator that preserves the original user’s intent. We evaluate agents as collaborators, not just patch generators: final pass rate and how many user interventions were needed to get there. Somehow also got wedged in right before Mark Cuban, so no pressure. We dug into why global benefits is the last giant pool of healthcare spend with almost no intelligence on top of it, and why AI is the first tech that actually thrives in that mess. Some of my favorite things we got into: 1️⃣ The American employer is floating the entire healthcare system. Roughly 1.5 to 1.7 trillion dollars, paying an average of 262% of Medicare, mostly flying blind. 2️⃣ Employers are the only players actually incentivized to keep you healthy. Health systems, plans, and PBMs make more money when people are sick. Your employer wants you well and productive. 3️⃣ Earn trust with a narrow wedge before you sell the platform. People think we sell ticket deflection. We don't. Deflection is the byproduct of people trusting the AI enough to go to it first. 4️⃣ Never ship an analysis a human can't act on. If it isn't actionable, it's a vanity metric. Prediction without a channel to act on is just a nicer report. 5️⃣ Meet people where work already happens. Nobody reads the benefits guide. They want a ChatGPT-style experience, so you show up in Slack and the tools they already live in. 6️⃣ Own your intelligence instead of renting it. The best benefits teams in five years will run like every other core function, testing a hypothesis continuously instead of buying a once-a-year report. 7️⃣ And the one that has nothing to do with software: prioritize the crystal ball moments and let the rubber balls bounce. Getting here took years of building, refining, and believing in a care model built on the belief that quality care should be simple and accessible. Every milestone along the way, every partnership built, every process improved, has led to this moment. This funding is not the finish line. It is fuel for what comes next: reaching more patients, building a stronger team, and continuing to prove that virtual care can be both accessible and exceptional. Sandhu liked thisToday, I'm extremely proud to share that Ognomy Sleep has raised a $20M Series A financing. This is a huge milestone for us and I’m still reeling in emotions! I spent most of my career taking care of people with sleep apnea and I’ve seen what happens when it’s ignored, minimized, or made just too hard to treat. I’ve also seen what happens when people finally get the right care they deserve. They feel so much better in many ways. To name a few, they think more clearly, their blood pressure comes down, their heart health improves, their partner finally sleeps through the night, they stop nodding off at stoplights, and somewhere along the way life just gets a little lighter. That is why we are building Ognomy Sleep – simply because sleep apnea care should not require months of waiting, multiple disconnected appointments, confusing handoffs, or patients having to become project managers of their own diagnosis. From the beginning, our mission has been simple: open up access to high-quality sleep apnea care for the people who need it. It should meet people where they are, which is usually at home, tired, and wondering why they feel so terrible. The numbers tell part of the story; we’ve achieved more than 85,000 visits on the platform where patients are seen 9x faster than the traditional system. Our patients didn't have to wait, drive hours, or navigate a broken system. Those numbers aren't just metrics we put in a slide deck to make investors comfortable…they are real people getting great care. We are still early in the journey. Smart people keep refining VBC models… but the real problem is the model itself. Value-based care treats patients as populations—risk buckets, attribution groups, Star-Rating cohorts. It’s fragile accounting, and ineffective medicine of the averages. The real leverage point isn’t better population mechanics. It’s precision medicine, n=1 care based on genomics, biomarkers, wearables, and real-time data streams. When medicine treats individuals instead of abstractions, outcomes improve without the financial gymnastics VBC requires. Another decade tuning shared-savings formulas is *not* what’s needed. Clinical and data systems built around the person in front of us, not the mean behind them, is the solution. Find out about the approach to #AI tools in clinical care at Penn Medicine, University of Pennsylvania Health System. Srinath Adusumalli, MD, MSHP, MBMI, FACC, vice president and chief #health information officer at the health system, shares valuable insights. With so many AI tool options becoming available in clinical care, it is important for senior leaders to have a focused approach for AI tool adoption, according to Adusumalli. "At the highest level, when we adopt AI tools in clinical care, we focus on the problems we are trying to solve," Adusumalli says. "Given the number and proliferation of AI tools, we feel it is important to have a clear understanding of what we are trying to accomplish for our patients, clinicians, and organization." Penn Medicine is adopting AI tools in clinical care that solve specific problems for patients and clinicians. For patients, a top goal is to use AI tools to partner with patients to help them find and understand health information, then interact with that information in a more dynamic way than patients could in the past, Adusumalli explains. "With AI tools that give patients insights, we want patients to be able to use those insights to access the right type of care at the right time," Adusumalli says. "That care could be at a brick-and-mortar setting, virtual care, or specialty care, and navigating access to the right care can be a cumbersome process for patients." One of the AI tools that Penn Medicine is evaluating in a new collaboration with K Health is designed to improve patient engagement with the Penn Medicine OnDemand service, which provides 24/7 virtual care. For clinicians, the health system is adopting AI tools that give clinicians and patients a running start in their interactions as well as improve the quality of those interactions, according to Adusumalli. "If patients gain insights from using an AI tool about their health, we want those insights to form the basis for a clinical visit," Adusumalli says. "Then we want to use AI tools such as AI scribes to operate in the background, so clinicians can put more of their attention on focusing on patients rather than swiveling back and forth with a computer." I’m interested in FHIR-native implementations of health-care workflows that are not primarily data analysis or interchange. Because Ottehr is a FHIR-native EHR, all our real-world workflows are FHIR-based. Front desk staff checks-in a patient on the tracking board, FHIR. Clinician orders a lab, documents a procedure, FHIR. Coding an encounter for claim submission, FHIR. Message sent to patient via SMS reminding them to complete paperwork for upcoming visit, FHIR. Writing a prescription, collecting a co-pay via credit card, and on and on. Of course there is a great deal of integration work under the hood to communicate with external systems using legacy formats, but we have a strong preference for working with tech-forward solutions. For example, we have a wonderful partner in AdvaHealth Solutions, which provides great FHIR APIs for sending radiology orders and retrieving results. Are there others? If you are implementing a clinical solution with a FHIR-first API that isn't just data interchange, we’d love to learn more and promote it.
