Product Manager – Value-Based Care & Patient Engagement
We are seeking a Product Manager to lead the strategy, development, and optimization of digital healthcare products that improve patient outcomes, drive engagement, and support value-based care initiatives. This individual will work closely with clinical, operational, engineering, analytics, and business stakeholders to build solutions that enhance the patient experience, improve care quality, and deliver measurable outcomes across healthcare populations. The ideal candidate brings deep healthcare domain expertise and a strong understanding of value-based care models, population health management, and patient engagement strategies.

Key Responsibilities
- Own and drive the product roadmap for patient engagement and value-based care initiatives.
- Partner with clinical, operations, analytics, and engineering teams to define product strategy and prioritize features.
- Leverage patient, provider, and population health data to identify opportunities to improve engagement, quality measures, and care outcomes.
- Define product requirements, user stories, success metrics, and business cases for new capabilities.
- Drive end-to-end product development from discovery and ideation through launch and optimization.
- Analyze user behavior, healthcare trends, and market dynamics to inform product decisions.
- Measure product performance through KPIs and implement continuous improvements based on data and stakeholder feedback.
- Ensure solutions align with regulatory requirements, healthcare compliance standards, and organizational objectives.
- Support initiatives focused on preventative care, care gap closure, patient outreach, care management, and quality improvement programs.
- Collaborate with cross-functional teams to deliver scalable digital experiences for patients, providers, and care teams.
Required Qualifications
- 8+ years of Product Management experience.
- Healthcare industry experience required.
- Experience supporting value-based care, population health, accountable care organizations (ACOs), care management programs, or payer-provider initiatives.
- Experience building patient-facing, provider-facing, or care team-focused digital products.
- Strong understanding of healthcare workflows and patient engagement strategies.
- Experience defining product roadmaps, managing backlogs, and translating business needs into product requirements.
- Strong analytical and data-driven decision-making skills.
- Excellent communication and stakeholder management abilities.
- Experience working within Agile product development environments.
Preferred Qualifications
- Experience developing products focused on patient engagement, preventative care, outreach programs, care gap closure, or member experience.
- Familiarity with value-based reimbursement models and healthcare performance metrics.
- Experience partnering with clinical, data science, analytics, or care management teams to deliver measurable healthcare outcomes.
