The Statewide Voice for Street Medicine

Launched in 2022, the California Street Medicine Collaborative brings together over 275 organizations — including providers, health plans, cities and countries, and people with lived expertise — to strengthen the field of street medicine statewide. Hosted by USC Street Medicine, we support the direct delivery of care to people living unsheltered — under bridges, in encampments, and on sidewalks — across California.
The California Street Medicine Collaborative is comprised of 275+ member organizations, 1000+ members, and has developed 80 programs in 31 countries.

What We Do
We unite diverse stakeholders to:

Transform the Healthcare System — Improve health access & equity for people who are unsheltered in California

Scale Street Medicine — Accelerate the development of street medicine programs across California

Build a Community of Practice — Collectively solve challenges facing street medicine and our patients

Improve Care — Identify, research, & promote practices & policies that improve care on the streets

Facilitate Exits from Homelessness — Leverage trusted relationships to create paths into housing
Our Policy Priorities
1. Integration of street medicine into the broader healthcare system. Policies & infrastructure that facilitate the integration of street medicine teams into hospitals, primary care clinics, behavioral health facilities, and other settings.
2. Policies and infrastructure that enable integrated care models for mental health and substance use. Promote and fund integrated care models that enable street medicine to collaborate with other services to address both physical and mental health needs, including SUD.
3. Policies and contracts that enable street medicine to directly access medically necessary services (MNS) for their patients, regardless of the PCP assignment.
4. Expanded funding and equitable rates to ensure sustainability. Street medicine teams deserve equitable rates and contracts to ensure sustainability, with flexible funding to enable care tailored to the context.
