LA GRANGE, KY- Resident fell from the back of a transport van when staff released the safety restraints before assuring that the hydraulic lift was in place.

AUSTIN, TX- Facility failed to ensure residents received necessary treatment and services, to prevent
new pressure ulcers from developing. LVN stated, “it was unacceptable what happened to Resident #1’s heel”.

WINDER, GA- The facility failed to ensure weekly skin assessments and wound observations were completed for R145 and failed to provide the recommended treatment for a sacral pressure ulcer.

CORSICANA, TX- The facility failed to ensure the residents had the right to be free from abuse and
neglect.

MCDONOUGH, GA- The facility failed to protect four resident’s right to be free from physical abuse.
